Preceded in death by wife, Helen Phillips ; brothers, Charles, Hubert, Bill and John Phillips ; step-grandchild, Tony Gulley. Survived by daughter and son-in-law, Vicki and Ron Monday; beloved grandchildren and spouse, Shea Dukes, Brandi and Cory Maples; stepdaughter and spouse, Ann and Curtis Gulley; step-grandchildren and spouse, Randy and Tammy Gulley and Cindy Mullendore; several step-great-grandchildren; sisters and spouses, Martha and Leonard France , Annie Henderson, Helen Gribble; several nieces and nephews. With his colorful personality, Jack never met a stranger. He had a host of friends of all ages with whom he shared many stories, laughter and memories and will be greatly missed by all.
